How to make an Access to Information or Privacy request

To make an Access to Information or Privacy request for information held by the Public Health Agency of Canada, you may complete the Access to Information or Privacy Request Form or send a letter mentioning the Act and describing the information you are seeking to the ATIP Office. Please include any relevant details to help the ATIP Office locate the information.

1. Formal Request:

Apply online today to save time and postage.

Mailing Request

Written requests may be submitted via:

• Treasury Board Secretariat's Access to Information Request Form (TBC/CTC 350-57);
• Treasury Board Secretariat's Personal Information Request Form (TBC/CTC 350-58);
• A signed and dated letter indicating that the request is made in accordance with the Act and describing the information you are seeking.

2. Proof that you have the right to make a request:

Access to Information Request

• To be eligible, you must be a Canadian citizen, a permanent resident of Canada or an individual or corporation currently present in Canada.

Personal information requests

• Everyone has the right to access their personal information held by government institutions that are subject to the Privacy Act.

3. Application fee

• There is a $5.00 application fee for requests made under the Access to Information Act.
• Payment can be made by:
  • Credit card (Visa, MasterCard or American Express) using the ATIP Online Request's payment service; or
  • Cheque or money order made payable to the Receiver General for Canada when applying by mail.
