Information for clients from Department of National Defence and Canadian Armed Forces

The Passport information billing form is not required for Department of National Defence (DND) and Canadian Armed Forces (CAF) clients; however, you must submit one of the following documents with the application:
For a special passport, submit the Requisition form for official passports for temporary duty/posting abroad for the Department of National Defence [PPTC 455] (PDF, 92 KB)
The requisition form must be completed in full and included with all applications for special passports by DND/CAF clients. The application must include:
- the purpose of the trip,
- the cities and countries that will be visited, the dates of travel, and
- the detailed flight itinerary.
This information is critical as it will permit Official Travel staff to clearly identify the type of document to be issued, its validity period, official observations to be included and whether any additional action is required (such as obtaining a visa).
For a diplomatic passport, submit the Department of National Defence posting confirmation form for diplomatic passports [PPTC 458] (PDF, 50.41 KB)
The posting confirmation form must be completed in full and included with all applications for diplomatic passports by DND/CAF clients.
