About the changes
We're making changes to the process for the Special Access Program (SAP) for medical devices. The changes will:
- reduce errors
- protect your information better
- create a clearer and more consistent process
- make it easier to apply for a medical device SAP authorization
The changes include introducing:
- an accessible application form
- a new secure method for submitting your information
New application form in accessible format
We have created a new accessible PDF application form.
The new form:
- is clearer and easier to use
- meets our accessibility standards
- will help us process applications more efficiently
New secure submission method
We have a new, secure method for submitting your SAP application.
The method:
- speeds up intake and processing
- keeps patient and clinical information secure during transmission
For the fastest processing time, we recommend using our secure submission tool to submit your application. If this isn't possible, you may fax the information to 613-957-1596.
We do not monitor our inboxes outside of regular business hours.
Timeline and steps to take
You may start using the new accessible application form and submission method now.
Steps to take:
- Download and start using the Application Form for Special Access Medical Devices (PDF) and our spreadsheet template.
- Submit your medical device SAP requests using our secure transmission tool (recommended) or by fax.
- Update your clinic workflows, templates or saved links that are now out of date.

As of February 1, 2027, Health Canada will only accept applications using the form and transmission methods described in this notice.
