Proof of death of a member of the public service pension plan

In the event of a member/former plan member's death, the pension centre for federal employees should be contacted immediately, and the death certificate issued by the funeral director showing the date of death should be sent along with the required supporting documentation that is listed in the following sections:

Plan member or former plan member with survivors (widow/er, partner, and/or children under age 25)

Required Information

  • Name of deceased
  • If pensioner is receiving survivor benefits, provide the second superannuation number
  • Name of person calling/writing
  • Relationship to deceased
  • Survivor's Social Insurance Number
  • Telephone no. for survivor
  • Language preference of survivor
  • Proof of death
  • Confirmation of children under the age of 25 and proof of age
  • Letter from survivor to request (if applicable):
    • Direct deposit
    • Public Service Health Care Plan (PSHCP)
    • Continued Dental Plan coverage
  • Return of all cheques issued after the month of death

The survivor's superannuation pension number will be the same as the annuitants and must be used in all communications with the Government of Canada pension centre.

Pensioner with no survivors

  • Name of person calling/writing
  • Name, address and telephone number of Executor of estate
  • Language preference
  • Proof of death
  • Written confirmation that there are no eligible survivors "legal spouse, common-law spouse, same-sex partner, children under the age of 18 and students between 18-25 years of age"
  • Return of all cheques issued after the month of death
