Checklists for security assessment applications
Use these checklists to ensure that you are meeting all of the requirements for completing security assessment applications.
Checklist for security assessment application of authorized individuals or an owner with 20% or more voting shares or interests
The authorized individual or owner must include the following documentation when submitting a security assessment application to the Controlled Goods Program:
- completed security assessment application
- a copy/scan of 2 pieces of government-issued identification (at minimum one must be photo identification)
- proof of citizenship (for example, birth certificate, passport, permanent resident card)
- proof of residence (for example, driver's licence, government-issued document with address)
- a certified criminal record check based on fingerprints or a name-based criminal record check verified against the Royal Canadian Mounted Police (RCMP)'s Canadian Police Information Centre (CPIC) system
Checklist for a security assessment application of a designated official
The designated official must include the following documentation when submitting a security assessment application to the Controlled Goods Program:
- completed security assessment application
- a copy/scan of 2 pieces of government-issued identification (at minimum one must be photo identification):
- proof of citizenship (for example, birth certificate, passport, permanent resident card)
- proof of residence (for example, driver's licence, government-issued document with address)
- a certified criminal record check based on fingerprints
- submit the certified criminal record check based on fingerprints report directly to the Controlled Goods Program. This report is available from your local police station, Royal Canadian Mounted Police office or private fingerprinting companies accredited by the RCMP
- in rural or remote areas, where fingerprinting services are not available, designated officials will need to submit an original Royal Canadian Mounted Police C216-C fingerprint identification form with the security assessment application
- the form will be forwarded to the Royal Canadian Mounted Police for processing, possibly causing delays in the completion of the security assessment
- Royal Canadian Mounted Police C216-C fingerprint identification form is available from your local police station or Royal Canadian Mounted Police office
- in rural or remote areas, where fingerprinting services are not available, designated officials will need to submit an original Royal Canadian Mounted Police C216-C fingerprint identification form with the security assessment application
- submit the certified criminal record check based on fingerprints report directly to the Controlled Goods Program. This report is available from your local police station, Royal Canadian Mounted Police office or private fingerprinting companies accredited by the RCMP
- certificate of good conduct from each country where the individual has resided for 6 consecutive months or more
- résumé (optional)
Checklist for security assessment application of employees, officers or directors who will examine, possess or transfer controlled goods (conducted by a designated official)
The designated official needs to collect the following documents to complete a security assessment of employees, officers or directors who examine, possess or transfer controlled goods:
- completed security assessment application
- a copy/scan of 2 pieces of government-issued identification. At least one must be photo identification
- proof of citizenship (for example, birth certificate, passport, permanent resident card)
- proof of residence (for example, driver's licence, government-issued document with address)
- a certified criminal record check based on fingerprints or a name-based criminal record check verified against the RCMP's Canadian Police Information Centre (CPIC) system
- credit check report (may be requested by designated official)
- certificate of good conduct from each country where the individual has resided for six consecutive months or more
- the completed security assessment summary by designated official conducting a security assessment of an employee, director or officer (PDF 180 KB) form
- the completed notice of security assessment authorization (PDF 191 KB) form or notice of security assessment denial (PDF 184 KB) form
This documentation is not submitted to the Controlled Goods Program, but must be kept on file. Only in high-risk cases does this documentation need to be sent to the Controlled Goods Program.
The Royal Canadian Mounted Police is phasing out their name-based criminal record check process in favour of a system that uses electronic fingerprints to identify individuals. For security assessments applications of designated officials submitted to the Controlled Goods Program, a certified criminal record check based on fingerprints is already mandatory for criminal records checks.
For security assessment applications of authorized individuals and owners, as well as for security assessments of employees by their company’s designated official(s), the Controlled Goods Program recommends that a certified criminal record check based on fingerprints be used to conduct criminal record checks.
