Complete this online form indicating:
- your full name
- your Personal Record Identifier (PRI), if you’re a public servant, or your service number if you’re a member of the Canadian Armed Forces (do not send your Social Insurance Number)
- name/number of test
- date of birth
Note: Confirmation of previous test results is only available for tests conducted by the Public Service Commission of Canada. Please allow at least 5 business days for us to process your request.
