Use this form if
- you applied for a Canadian citizenship certificate and it’s been more than 6 weeks since we mailed it to you, but you haven’t received it
- If it’s been more than 180 days, you must apply for a new certificate.
- To find out when it was mailed, check your file online.
or
- your Canadian citizenship certificate was lost, stolen or destroyed
- You must submit a declaration, even if you don’t plan to replace it.
- We recommend you apply for a new citizenship certificate.
