Use this form if you want to allow Immigration, Refugees and Citizenship Canada (IRCC) to release your personal information to someone you choose. The person you choose will be able to get information on your case file, such as the status of your application.
They will not be a representative who can do business for you with IRCC. A person must be appointed as a representative to do business for you with IRCC.
If you have not yet sent your application, send this form with your application to the office listed in your application package.
If you already sent your application, send this form to the office where you sent your original application.
