3. How to register to the NDR portal and access your dose history summary report

  1. If you are coming directly from Section 2 above, go directly to step 5. If you already have your GCKey and are not accessing the portal for the first time, follow the next steps to reach the NDR portal.
  2. Click on the link https://rap-par.hc-sc.gc.ca/ords/f?p=500. On the language selection screen, click on your preferred language button.
  3. On the next screen, click 'Continue to GCKey'.
  4. On the 'Welcome to GCKey' screen, enter your GCKey Username and Password, then click the 'Sign-In' button.
  5. You will be directed to the NDR Portal Enrollment screen. Review the Eligibility Details, check the box to agree to the terms and conditions, and click the 'Apply Now' button.
  6. Complete the 'National Dose Registry (NDR) – Access Request Application' form by entering your First Name, Last Name, SIN, Date of Birth, and Personal Email Address, then click 'Submit'. If you have an activation code (provided by the NDR staff), click on 'I have an activation code'.
  7. Wait for your application to be processed. Be patient, this can take some time. Do not refresh the page. You will receive a confirmation or rejection email when the process is complete. If your information is incorrect or if you don't have records in the NDR database, your application will be rejected.
  8. You will be presented with the National Dose Registry (NDR) – Account pending screen. Click on 'OK'.
  9. Click on 'NDR Portal' on the following screen to access the NDR portal, where you can access your Dose Report (if your application is successful).
  10. You will land on the NDR Portal main page (Home). You can always come back to this page by clicking 'Menu' and selecting 'Home'.
  11. To download your Dose History Summary report in PDF format, click on 'Proceed to dose history summary report'. Alternatively, click on 'Menu', select 'Radiation Worker' and then 'Download dose history summary (PDF)'.
  12. Click on the 'Dose History Summary' button of your choice to download a copy of your Dose Report. If your work falls under the Canadian Nuclear Safety Commission's (CNSC) jurisdiction, you will likely need the CNSC Text report. Otherwise, choose the DHS text report (also known as the Personal Dose History Summary report). Confirm with your employer which report is required.
  13. You can open the Employee Dose History Report by clicking on the link in your 'Downloads' folder.
  14. The 'Menu' also offers an accessible HTML version of the Dose History Summary Report. To access it, click on 'Menu', select 'Radiation Worker' and then 'View dose history summary (html accessible)'. In the 'report' box, pick the type of report you want to see (see step 11 above). In the 'report detail' box, select the information you want to view.
  15. Once you are ready to log out, click on 'Menu' and select 'Sign out'.

Please note that the system will automatically log you out of the portal after 15 minutes of inactivity. You will then have to log in again using the link provided at the beginning of this guide (you may need to close and restart your browser).
