About this form

The following web form can be used by Canadian importers to fulfill the notification requirements prescribed in subsections 5(1) and (2) of the Exemption Order in Respect of Foods for a Special Dietary Purpose (the Order). To learn more about the Order and the exceptional importation and sale of certain foods, please consult Part 1 of the Guide to the exceptional importation and sale of certain foods for a special dietary purpose (FSDP) in response to a shortage or risk of a shortage.

A single notification form may be used for a shipment consisting of 1 to 5 different product varieties (e.g., products of distinct types, sizes, formats, flavours, etc.). Note – all product varieties must be part of the same shipment and arrive in Canada at the same date and port of entry.

How to complete the form

Follow the 3 steps below to complete your 'Notification of importation form'. Whether you are submitting information for a single product or multiple products, please ensure to provide all the required information. Once you completed the form, please submit it to Health Canada by pressing the 'Submit' button located at the end of the form.

Step 1 - Product information

Product information 1

1. The brand name and product name of the food being imported for exceptional sale in Canada (required)

2. The category of food set out in column 1, Part A of the List to which the food for exceptional importation and sale belong (required)

3. The Foreign Regulatory Authority set out in column 2, Part A of the List that has authorized the sale of the food (required)

4. The jurisdiction set out in column 3, Part A of the List from where the food is imported (required)

5. The name and contact information of the foreign manufacturer (as it appears on the food's label) (required)

6.The estimated number of units of the food(s) in the shipment (e.g., five hundred 250 ml bottles) (required)

Note – If your shipment exceeds five product varieties, a supplementary Notification of importation form will be required for the additional varieties.
