The personal information you provide to the Tobacco Control Directorate & Tobacco and Vaping Compliance and Enforcement Program will be used by the program under the authority of section 4 of the Department of Health Act, the Tobacco and Vaping Products Act, and handled according to the Privacy Act.
Collecting your personal information
We collect your personal information to contact you to respond to your queries and comments. It may also be used for compliance and enforcement activities.
Other reasons we may use or share your personal information
In limited and specific situations, your personal information may be disclosed without your consent in accordance with subsection 8(2) of the Privacy Act.
Providing your personal information
You must provide the personal information requested on this form to allow the Program to respond to your question or concern, follow up on a complaint and complete an investigation. Sensitive personal information, such as your Social Insurance Number, is not required and should not be included in your message.
Know your rights
You have the right to access and request a correction and/or notation to your personal information. You also have a right to complain to the Privacy Commissioner of Canada if you feel your personal information has been handled improperly.
