Printable forms: Solvent Degreasing Regulations

Please note that if you import PERC for any purpose or sell for use by dry cleaners you may also have reporting requirements under the Tetrachloroethylene (Use in Dry Cleaning and Reporting Requirements) Regulations.

These forms are being offered as an on-line service and are available in multiple formats; however, a signature is required on whichever format is chosen. Once you access the forms, you must print them and fill in the fields for submission by mail. Forms can be mailed to:

  This form must be used to make the annual solvent consumption report under Paragraph 7(a) and Schedule 6 of the Regulations.

  This form must be used to make a consumption unit retirement notice under Paragraph 5(1) and Schedule 4 of the Regulations. A separate consumption unit retirement notice must be made for consumption units issued for a given solvent and a specific degreasing process.

  This form must be used to make a joint application for a consumption unit transfer under Paragraph 6(3) and Schedule 5 of the Regulations. A separate joint application for the transfer of consumption units must be made for a given solvent and a specific degreasing process.
