3. Telcon Parent/Guardian – Date

1. Pertinent details to include:
   1. rank, name;
   2. home address and phone number;
   3. corps/squadron number;
   4. course name;
   5. date of RTU;
   6. travel arrangements (include Itinerary, method of travel, and booking reference numbers);
   7. reason for RTU;
   8. Z Trg O notified (rank, name and date);
   9. Corps/squadron CO notified (rank, name and date);
   10. parent/guardian notified (name and date);
   11. remarks (Travel supervisor needed, follow-up required, any other info);
   12. letter containing complete explanation of RTU to follow; and
   13. financial consideration (trg bonus paid or not, scdt pay).
