COVID-19 Information for Canadian Armed Forces members in the National Capital Region (NCR)

COVID-19 Vaccination

COVID-19 vaccines can be obtained through CF H Svcs C (Ottawa) by booking an appointment via the booking tool: http://10.97.166.62/apps/opsoft/apps/vax/index.php (only accessible through DWAN).

The clinic is by appointment only. No walk-in offered.

If you do not have DWAN access: please contact ImmunizationNCR-ImmunisationRCN@forces.gc.ca and provide the following details:

• Name
• SN
• Date of birth
• Personal Email
• Phone
• Name of vaccine previously received (Mod/Pfi/AZ/Jan)
• Date of last dose

Vaccines offered:

• Spikevax bivalent
• Comirnaty Bivalent (Pfizer)

Admissibility: As per NACI Updated guidance on COVID-19 vaccine booster doses in Canada, it is recommended that a COVID-19 booster dose be given at 6 months after a previous COVID-19 vaccine dose or after a previous COVID-19 infection. However, an individual may be administered a COVID-19 booster as early as 3 months following a COVID-19 infection, depending on their individual risk of COVID-19 infection, local COVID-19 epidemiology, and operational considerations.

When: Every Thursday afternoon from 13:00 to 14:30

If you received a COVID-19 Vaccine on the civilian side: Please send your proof of vaccination in pdf format and the following information to: ImmunizationNCR-ImmunisationRCN@forces.gc.ca.

• First Name
• Last name
• Date of Birth
• Home email address
• Vaccine Name
• Administration date

The immunization team will update your medical records and you will receive your QR Code through DWAN.
