• Commissioner’s Directive (CD) 041 – Incident Investigations

This policy was modified as a result of the Commissioner’s Directives Standardization Project. Amendments have been made to clarify official languages requirements and to standardize existing practices. In addition, an amendment has been made in response to the passing of Bill C-83, An Act to amend the Corrections and Conditional Release Act and another Act.

Modifications to the policy document include:

• a new look, resulting in stand-alone English and French versions
• amendments to ensure CSC meets legislative requirements with respect to the Official Languages Act
• clarification to definitions of “file review” and “local investigation”
• amendment in the time requirement for submitting a Situation Report from three days to five business days to mirror the Situation Report Guidelines
• the term “mortality review” has been replaced by “quality of care review” in accordance with the passing of Bill C-83. National quality of care reviews will be formally closed via a closure memo by the Assistant Commissioner, Health Services, rather than by the Senior Deputy Commissioner.

The policy was amended by the Incident Investigations Branch in collaboration with the Strategic Policy Division, and with input from internal and external stakeholders.
