• Commissioner’s Directive (CD) 568 -1- Recording and Reporting of Security Incidents

The Preventive Security and Intelligence Branch has amended Annex C of CD 568-1- Recording and Reporting of Security Incidents to align with the reporting and recording requirements outlined in the new CD 574 - Sexual Coercion and Violence Framework.

This change will enhance the Service’s ability to report on incidents of sexual coercion and violence to internal and external stakeholders.

This change is effective immediately and will be incorporated into the forthcoming CD revisions.

The change made to Annex C of CD 568-1- Recording and Reporting of Security Incidents is as follows:

• Allegation of sexual assault is now a reportable institutional incident under Annex C - Incidents to be Reported Immediately.

The Preventive Security and Intelligence Branch developed this interim policy bulletin in collaboration with the Security Operations Division and the Strategic Policy Division.

All staff responsible for reporting and recording security incidents as per CD 568-1.

Accountabilities are listed in the policy document.

To allow for Security Intelligence Officers/ Parole Officer Supervisors or delegate to record an allegation of sexual assault, a new incident sub-type “Sexual Assault Allegation” has been added to the Incident Report Module of the Offender Management System.

Reportable Institutional Incidents

• All complete lockdowns of an institution
• All deaths (natural causes, unknown causes, and suicide)
• Any incident involving a child participating in the residential Mother-Child Program at a women offender institution
• Any incident involving serious bodily injury (SBI)
• Any incident that requires activation of the Institutional Crisis Information Centre
• Any incident which has attracted significant media and/or ministerial attention or is likely to
• Any medical emergency involving hospitalization and a life-threatening situation
• Assault on a staff member/volunteer/contractor/visitor
• Assault on an inmate that requires outside medical attention
• Attempted escape
• Attempted suicide
• Body cavity search
• Discovery of firearms, explosives and/or lethal substances
• Escape and being at large without excuse
• Escape from CSC escort, staff or citizen, excluding Community Correctional Centres
• Exceptional search (section 53 of the Corrections and Conditional Release Act)
• Failure to return from an unescorted temporary absence
• Failure to return from a temporary absence/work release or an unescorted temporary absence
• Failure to return from a work release
• Hostage taking, forcible confinement, forcible confinement with sexual assault
• Information technology related incident (for more details regarding procedures to follow, please refer to CD 225 – Information Technology Security and CD 226 – Use of Electronic Resources)
• Intentional discharge of a firearm
• Major disruption
• Major disturbance (in an institution or other CSC facility/workplace)
• Murder or attempted murder by or of an inmate
• Prison breach, excluding Community Correctional Centres
• Sexual assault and/or allegation of sexual assault (for more details regarding procedures to follow, please refer to CD 574 – Sexual Coercion and Violence Framework)
• Staff caused disruption of day-to-day activities in the institution and/or community (this includes, but is not limited to, all labour activities such as information pickets, strike activity and job actions and/or sections 127 and 128 of the Canada Labour Code activities)
• Suicide/violent death of an employee (during or outside working hours)
• Threats towards staff/volunteers/contractors (verbally and physically)
• Unmanned air vehicles flying over or near the institutional property

Reportable Community Incidents

• All deaths (natural causes, unknown causes, and suicide)
• Any incident involving serious bodily injury of an offender occurring in a Community-Based Residential Facility
• Any incident where staff/volunteer/contractor safety has been put at immediate risk and which requires police involvement (see Employee Protection Program)
• Any incident which has the potential to attract or has attracted significant media and/or ministerial attention
• Attempted suicide
• Hostage taking, forcible confinement, forcible confinement with sexual assault
• Incident with significant police involvement
• Information technology related incident (for more details regarding procedures to follow, please refer to CD 225 – Information Technology Security and CD 226 – Use of Electronic Resources)
• Major disturbance in a CSC facility/workplace, parole office, Community Correctional Centre, and/or Community Residential Facility
• Murder or attempted murder by or of an offender
